New Student Enrollment Requirements
Enrollment of new students can be completed in the school office. Returning students will fill out preenrollment forms during each spring semester. State law and district policy requires that parents bring the following information with them at the time of enrollment:
• Official birth certificate for all new students enrolling in MPS
• two (2) proofs of residence (excluding telephone, cablevision bills, driver's license and personal checks)
• child's social security number (optional)
• immunization record.
STUDENTS MAY NOT ENROLL WITHOUT COMPLETE IMMUNIZATION RECORDS OR PROOF THAT THE STUDENT IS IN THE PROCESS OF RECEIVING REQUIRED IMMUNIZATIONS. NO EXCEPTIONS WILL BE MADE. Exemptions to the immunization law may be granted in accordance with the state law and State Health Department requirements. |
